If you move, change your phone number or change your name while you have a New York workers’ comp case, the change has to reach the insurance carrier, not just the Board. The form for that is the OC-966, Claimant Name and Address Form. Getting it right matters: hearing notices, decisions and sometimes your checks go to the address on file.
Why the carrier has to update your address
The insurance carrier sends the Board electronic filings throughout your case. Those filings automatically update your address in the Board’s file, and they overwrite any address the Board already had. So if you tell only the Board about a move, the carrier’s next filing can put your old address right back.
To fix that, the Workers’ Compensation Board created the OC-966 in December 2024. The form goes to the carrier, and the carrier then sends the Board an electronic filing that updates your name or address.
What the form asks for
- Your previous name, mailing address, phone and email
- Your new name, mailing address, phone and email
- Your WCB case number and date of injury
- The carrier’s claim number, and the insurer’s name and mailing address
- Your attorney’s information, if you have a lawyer
Where to send it
- Send it to the insurance carrier, its third-party administrator, or your employer if it is self-insured.
- Do not send it to the Board. The form itself says so.
- Keep a copy, with the date you sent it. It is your proof that you asked for the change.
If you have a lawyer, your lawyer sends the OC-966 and certifies that a copy went to the carrier. Since March 2026, if the carrier has not updated your information within 15 days, your attorney can report it to the Board at a dedicated email address. Carriers that do not promptly update your information may be penalized.
Other people to tell
- Your doctors, so their reports and bills carry your current information.
- Your lawyer, so nothing about your case goes to an old address.
- Your employer’s payroll or HR, if you are still employed there.

Frequently asked questions
What is Form OC-966?
It is the Workers’ Compensation Board’s Claimant Name and Address Form. It is used to change an injured worker’s name or mailing address on a New York workers’ comp case.
Where do I send Form OC-966?
Send it to the workers’ comp insurance carrier, its third-party administrator or the self-insured employer. Do not send it to the Board. The carrier then updates your information with the Board electronically.
Why did my old address come back after I updated it?
The carrier’s electronic filings automatically overwrite the address in the Board’s file. If the carrier still has your old address, its next filing can replace your new one. That is why the OC-966 goes to the carrier.
What if the carrier doesn’t update my address?
Keep a copy of the OC-966 and the date you sent it. If you have a lawyer, your lawyer can report it to the Board if the carrier has not updated your information within 15 days.
